You’ll receive your NDIS plan with your approved funding and supports listed. You can now start using services to help reach your goals.

You can reach out to your LAC or EC partner and book in a meeting for them to explain your plan. They can help you:

Please note: your LAC or EC partner can’t recommend specific providers, as this would be a conflict of interest. But they can help you understand your options and answer any questions.

You can contact them at any time if you:

  • need help using your plan
  • want to make a change
  • have a question or concern about your supports

Getting ready for your plan review

Most NDIS plans now last up to 5 years, but that doesn’t mean you have to wait that long to ask for a review.

Mitochondrial disease (mito) is a progressive condition, and your needs may change over time. If your function or goals change, you can ask for a plan reassessment earlier. Contact your LAC, EC partner, or the Mito Foundation if you think your plan no longer meets your needs.

The NDIS will also contact you around 3 months before your plan ends. This gives you time to get ready for a review.

What to think about before a plan review

 

It helps to reflect on how things have changed since your last plan. Ask yourself:

  • Has your daily function changed? (e.g. Can you still prepare meals, drive, manage personal care?)
  • Are there new supports you need?
  • Have you stopped doing activities you used to enjoy?
  • Has your mobility, energy, or communication changed?
  • Do you need help at home, with transport or in the community?
  • Are your current supports helping you reach your goals?
  • Has your social life or independence changed?

You can begin collecting:

  • updated reports or letters from health professionals
  • notes on your daily routine or support needs
  • examples of what’s become harder or what’s helped
  • any new goals you would like to add
