Our dedicated Pathways Telehealth Nurse Michele will work with you to develop a tailor-made care plan to provide support, education and connection with other services. This telephone-based program is designed to help you navigate the health system and access all that is available to you.
Mito Foundation’s Pathways Telehealth Nurse Program offers personalised information and support to individuals diagnosed with mitochondrial disease (mito) and their carers, regardless of location.

What is the Pathways Telehealth Nurse Program?
The Pathways Telehealth Nurse Program provides a tailored review of individual health supports; identifying gaps, connecting to new services and developing tools for ongoing support.
The program will support you by:
- Conducting 2 – 3 short consultations to discuss your medical history, treatment and care requirements to understand your current needs and identify opportunities to improve your health care
- Guiding you through the health care system to access relevant services
- Strengthening your knowledge and confidence about your mito to proactively manage your condition
- Helping to educate your care team, including allied services and carers, about your mito and what care you need
- Coordinating access to health care and social support organisations
- Assisting with support at high needs times e.g. following hospital admission, discharge, and accident and/or emergency visits
It is important to note that the program will not provide treatment advice. The main objective is to develop a tailored care plan, provide practical and emotional support, and provide education and connection with health care and community services.

Meet Michele Hemmings
Michele has been in this role with Mito Foundation since 2019 and has developed a deep understanding of the needs of an individual and their families impacted by mito. Michele has over 25 years of experience in nursing and has helped many people navigate the complexities of the health care system. She has a diverse background across paediatric, adolescent and family centred care with extensive experience in chronic healthcare.
