Living with a rare, progressive condition can mean your disability support needs change over time. Change can be slow, and hard to notice, but can be serious.

These resources help you recognise change, describe it, and think about what support may help. Use them for yourself, your child, or someone you care for. Use them on your own, or with someone you trust.

Start with the resource that fits where you are now. Use it in small parts, over time. You can share parts with your disability support services, health care team, or people helping you. They will support you, so you do not have to explain the same things again and again.

More about these resources

What these resources are not

These resources do not cover everything. They focus on noticing and describing change.

  • They are not medical advice, legal advice, or a clinical assessment.
  • They cannot guarantee support, funding, or a specific outcome.
  • They do not replace advice from health, disability, legal, or advocacy professionals.

If you are worried about a sudden or serious change in health, contact your GP, health care team, or emergency services.

How these resources were developed

These resources were developed by Mito Foundation as part of the Rare Disease Disability Toolkit. The resources were developed in collaboration with rare, progressive disability communities through surveys, workshops and reviews at each stage of resource development. The Childhood Dementia Initiative, Children’s Tumour Foundation, Connective Tissue Disorders Network Australia and Fragile X Association of Australia contributed to consultation and content creation.

Quotes from people with lived experience with rare, progressive conditions were gathered throughout the project to show shared experiences. They are shared in these resources without names to protect privacy.

How to read the resources in other languages

Microsoft Edge can translate any text from a PDF document to a different language. You can also choose to have documents read aloud.
